United Community Care Alliances Inc.’s Community Care Alliance Initiative is an innovative “high tech and high touch” approach to solving health and social issues in underserved communities and neighborhoods.  The Community Care Alliance is shaped around the mobilization of existing, but fragmented resources that exist in every community. United Community Care Alliances Inc.’s Community Care Alliances are designed to bring to the surface access to untapped community support. Through our research, it was discovered that there are typically over 1,200+ non-residential buildings in every populated zip code with more than 15,000 residents. Imagine if every establishment (1,200+) in a community participated in their physical space and virtually to address the needs and issues of that community. Each establishment will be a community partner.  The establishment will be categorized into three groups we call Healthy, Wealthy, & Wise coalitions. These establishments are already for-profits businesses, non-profits, churches, government, community, and civic entities.  In person, these establishments would all share the same messages, resources, and Care Information stands and kiosks for direct community to access solutions for challenges people of these communities may be facing. Also, each establishment’s employees and customers can participate in the Community Care Alliances.
In person and on-line, the Community Care Alliances has a set of symbols/logos that will be displayed by every participating establishment. The Community Care Alliance logos will become immediate identifiers of solutions, relationships, resources, and hope for all in need of support. The logos will identify the Healthy (all mental/physical health professionals), Wealthy (all financial experts) and Wise (all education/training professionals) who have joined the alliance to assist in the growth and development of every community.
Emerging culture
United Community Care Alliances Inc. has an intercultural focus for developing communities. Every community is an eclectic mix of local, historical, artistic, talented, and ethnic expressions within itself.  United Community Care Alliances Inc. has many ideas on highlighting the emerging culture of expression and unity. United Community Care Alliances Inc. has planned to capture the wonder of every community through a video series of virtual diaries and mentoring. Scientific and Technological
Why guess at something, when the data can tell you what to do. United Community Care Alliances Inc. collects lifestyle data from and with every participant in the Community Care Alliance through the utilization of several tools (DayWheel, Vision Quest, readiness assessment, kingstreets.com, compassaimsolutions.com, etc.) This data is used to adapt the Healthy, Wealthy, & Wise Coalitions resources and relationships to the right people in need at the moment they need help and support. The DayWheel, Vision Quest, Kingstreets, and Compassaimsolutions.com are existing software/apps/websites designed to help with total daily life management. Each of these tools has a unique way of helping individuals to express and capture the perspectives of their own lives (we do not want to tell people what to do, we want them to tell us how to help).
The United Community Care Alliances Inc.’s Community Care Alliance and the Healthy, Wealthy, & Wise software/apps/websites will serve as the first place anyone should start when seeking support on progressing challenges in their lives and communities.
Currently, the Kingstreets website (https://www.kingstreets.com ) exists and showcases the potential for accessing direct resources through 10 database portals that are available to individuals and groups 24 hours a day, 365 days a year. The Organizational Operations software is available. It is intended to help United Community Care Alliances Inc. and its community partners to organize and segment all operation’s activities into a dynamic organizational chart. This allows organizations to quickly visualize and access information on every stakeholder in the Community Care Alliance. There are more technology tools being designed and will be released as the need arises. 
Environmental
United Community Care Alliances Inc.’s concern for the health and welfare of all its Community Care Alliances is paramount to United Community Care Alliances Inc.’s mission. Environmental concerns for underserved communities play a major role in the contributing factors to health disparities and inequities within underserved communities. Nationally, underserved communities across the country have a history of environmental mishaps that are still pervasive in most populated and rural communities. United Community Care Alliances Inc., through its social entrepreneurship, plans to highlight organizations and causes that encourage people to advocate on their behalf for environmental changes in their community.
Economical and health trends that can help to build a Culture of Health in the United States. 
In the new normal of COVID-19, through United Community Care Alliances Inc. services of consulting, professional development, and social entrepreneurship, the United Community Care Alliances Inc. plans to assist health/medical institutions and organizations get more accurate lifestyle and mental health data from all Community Care Alliance partners. COVID-19 has made working from home the standard of knowledge workers and changed the labor force forever.  The United Community Care Alliances Inc.’s consulting services is suggesting businesses will need to connect even more deliberately to their customers than ever before.  The Community Care Alliance and the Healthy, Wealthy, & Wise Coalitions will give businesses the opportunity to connect better to their existing and future clients.  Collectively, these strategies provide United Community Care Alliances Inc. with a high level sustainability, revenue, and scale.

How do you help everyone? Get everyone to help. Health disparities do not typically start inside of families and communities; they are caused by external removal of health resources, education, and experts in the surrounding community. Lack of access to health solutions (i.e., women services, youth services, elderly and aging services, mental health services, adequate food and nutrient options, inconvenient or non-existent medical facilities, and environment grievances, etc.).
In most cases, underserved and neglected communities suffer from redirected resources; not from its citizens being derelict in their concerns for their own health. More often than not, and so far from the truth, the assumption is underserved and neglected communities don’t want any help, don’t appreciate help, or don’t take advantage of the help offered. It is almost impossible to solve problems from a distance. If you are not willing to sit at the table with the people you are trying to help, why would they believe you care. From experience we know, if we go to them, they will come to us!
Take an existing idea and give it a new spin or novel application
People don’t care, until they know you care! This saying absolutely holds true. Alliances and coalitions everywhere are trying to solve big problems. How do you mobilize the mobilized? How do you organize all these alliances and coalitions into one unified force? How do you get every community to provide for itself? How do you turn on and cultivate all untapped resources, solutions, and providers within a community? There is an answer… United Community Care Alliances Inc.’s Community Care Alliances, Healthy, Wealthy & Wise Coalitions, and Champions bring everyone to the same table.
United Community Care Alliances Inc., with mobilized and coordinated communities, has strategies to bring services within individuals and communities’ comfort and convenient zones. At any time, no one should be no more than a few blocks away from front line resources and relationships, hence their convenience zone. Ultimately, problems will be eliminated or dealt with directly by being part of the daily living and movements of individuals. Healthy and affluent families surround family members with 24 hours of care monitoring. The same 24 hours of care monitoring should be available for individuals in struggling communities.
Apply ideas from other fields
The Urberizing of Care—Imagine using every industry, professional field, sector of commerce, city, and community to solve community problems together. United Community Care Alliances Inc.’s Healthy, Wealthy, & Wise Coalitions will do exactly that. Imaging every health, finance, education professional and entity contributing some asset (e.g., insights, expertise, time, information, knowledge, access, resources, relationships, funds, coaching, mentoring, guidance, and leadership) to uplift the quality of life for everyone. Yet, each professional and entity will only have to give what they can contribute; hence, a coordinated community. Simply, “every chance you get, share”.
The Community Care Alliance identifiers (Help Near Me): Logos, shields, wrist bands, care boxes, website and apps.
The franchising of care—Every coalition and alliance member will be identified by a Community Care Alliance wrist band with the area of support they are willing to provide to the Alliance (Healthy, Wealthy and/or Wise wristband). For example, if they come from the health field, they will have health colored wrist band. Through public interaction and through marketing, the communities will know it is ok to engage someone with this band for health inquiries. A professional with this band is not expected to treat or provide direct services, but to direct the person in the community to local services. This process would work exactly the same for those who have financial and education experience.
For individuals’ car, he or she will be able to display Community Care Alliances magnetic logo shield(s) on their car. The shield(s) will alert individuals in need that the person with the car can be approached for inquiry. United Community Care Alliances Inc. will monitor all interactions through the app. United Community Care Alliances Inc. will constantly look for best practices, share results, and outcomes. This will allow professional of United Community Care Alliances Inc. to life coach and mentor every person in every community. Every person in a community will have access to a minimum of one to three life coaches (health, wealth, wise-personal development coaches) if they so desire.

The future is shaped by what is happening and what we do now. The “normal” has changed forever. COVID-19 and the “Black Lives Matter Movement” has reshaped and reshuffled the world. Because of COVID-19 and Black Lives Matter, society has rushed into the future. The future of work has changed substantially. Cities will no longer ignore the cries of its underserved citizens. City and community managers have to change how cities have to address the needs of communities in their everyday living. The future of work will be a telework and knowledge-based workforce. Everyone is being forced to learn how to work and educate from home. Communities will coalesce around direct neighborhood issues, representation, and protection. Communities will have the technology and bandwidth to do practically everything from their homes and local community. Everyone deserves a courage and survival badge after 2020 and 2021. One size does not fit all, United Community Care Alliances Inc. is positioned to advance health and social equity by bringing “Quality of Life” solutions to every community.  At its fullest, for every community, health and social disparity will be addressed at the level in which disparities interrupt “quality of life” for individuals in every community.  United Community Care Alliances Inc. is structured to bring substantial resource to confronting health and social disparities.

Their needs become the center piece of the Community Care Alliances. Meeting their daily challenges and issues are the life blood of the mission of United Community Care Alliances Inc. desire to help transform communities.

Healthy families, healthy communities— United Community Care Alliances Inc.’s approach to community development will quickly become the standard for balanced, healthy, and sustainable communities. Every Care Alliance community will have a 12-20 year plan.  The plan allows United Community Care Alliances Inc. to go through a full cycle of a family conceiving a child, watching that same child graduate from high school, and receiving an advance degree (hence 12-20 years). Healthy families have healthy habits for life. United Community Care Alliances Inc.’s “community building” approach is to assist the community in assisting itself for longevity from one generation to the next. United Community Care Alliances Inc. would rather collaborate than compete and teach communities how to do the same.
The future is being shaped by technology and scientific advances in all disciplines. United Community Care Alliances Inc. plans to enlist professionals and subject matter experts to advise United Community Care Alliances Inc. on cutting edge practices that can be offered to participants in the Care Alliance. Through this coordinated effort communities will be prevented from lagging behind in health and social development.

Bold actions, bold results— United Community Care Alliances Inc.’s target is underserved and neglected communities around the country. United Community Care Alliances Inc.’s initial launch plans to deploy the Care Alliance in the 1000+ cities that have a street named after Dr. Martin Luther King, Jr. (https://www.kingstreets.com). Financial institutions typically classify these communities as Low to Moderate Income (LMIs) communities and designate programs to assist in the revitalization of these communities. A United Community Care Alliances Inc. goal is to have these cities and communities united in solutions, love, and legacy. United Community Care Alliances Inc. seeks to support policies that leaves no one behind, not just the child.  United Community Care Alliances Inc. will constantly pursue comprehensive policies that repair, build, assist, individuals and families.
• State any hypotheses or assumptions that undergird your work
Mobilized resources, produce productive resources—United Community Care Alliances Inc.’s theory and strategy, hypothesis, assumption is no real change happens unless you are willing to sit across the table from someone.  The primary hypothesis is that there are existing untapped resources and relationship in every underserved community. A secondary hypothesis is that everyone wants help. But the community (not just United Community Care Alliances Inc.) has to bring the help to them to make the resources accessible and available. Specifically, the help and resources must be in a person’s comfort and convenience zones. The help has to be close enough for them to access it in less than a few hours and within a few blocks or miles. United Community Care Alliances Inc. can’t expect people to have to travel all over the city to find the help they need.  United Community Care Alliances Inc. will bring the help to them. An assumption is that there are enough like-minded caring individuals to help communities become viable and self-sustainable. 
• Give us a clear idea of the activities grant funding would support and how they will be achieved;
Scale, Scope, and Pace—The grant funding is needed to expand the technological infrastructure (database, assessment, analytics, apps, etc.) to a size prepared to receive an enrollment of the majority of members of any given community. The apps will be primarily public facing. Which means the public will be interacting with all apps 24 hours a day to get services and to share experiences. United Community Care Alliances Inc. currently has multiple technologies, software development teams, and one app (www.mydaywheel.com). United Community Care Alliances Inc. may have to acquire 3rd party software and apps. 
United Community Care Alliances Inc. will need funding to train and orient its partners and champions on how to participate in the Community Care Alliance. Funding will be needed to produce the Community Care Alliances logos, shields, car magnets, and arm bands. Funding will be needed for marketing and promotions in the various participating cities. United Community Care Alliances Inc. will need trainers, project management professionals, and marketing professionals to design community engagement strategies. United Community Care Alliances Inc. will need funding to pay for updated and current research data for each alliance city. Some travel expenses will need to be covered.
Grant funding will be needed for professional services necessary for this undertaking. United Community Care Alliances Inc. will need program managers for deploying programs to various cities that do not have their own program managers. United Community Care Alliances Inc. will have an entire edutainment series and will require funding to secure entertainers, artist, musicians, actors, etc. to participate in events and activities around the country. Eventually, the United Community Care Alliances Inc. will need regional or local offices to conduct meetings and presentations. With an expectation of volume, United Community Care Alliances Inc. will need additional legal services for intellectual property and contracts generated in all markets. Additionally, accountants will be needed to assist with accounting needs in various cities. United Community Care Alliances Inc. anticipates joining local and regional organizations to build resource networks in every city. United Community Care Alliances Inc. may have to acquire certifications and bonding in certain cities, as well.

Change has to be managed—United Community Care Alliances Inc.’s mission and goal is to assist communities to configure, transform, and evolve into models of holistic, collective, and comprehensive healthy communities (mentally and physically). In football, there are 11 players with one ultimate goal as a team… to win. United Community Care Alliances Inc.’s goal is to help communities win. United Community Care Alliances Inc.’s goal is to show communities they have all the right players, the community just has to put them in the game. 

Every community is an orchestra with many musicians and one conductor. United Community Care Alliances Inc., using processes, systems, and a Vision Mapping methodology, plans to support communities by choreographing all the moving parts in a community into one succinct movement. The health, wealth, and wisdom of communities (Healthy, Wealthy, & Wise) already exist, but not in a collaborative presence. United Community Care Alliances Inc. brings all these resources into one mobilized force.

Rising tides raises all boats—United Community Care Alliances Inc.’s goal is to help produce self-sustainable Healthy, Wealthy, & Wise communities. Using methods derived from some of the best enterprise supply chains and logistics processes that maintain some of the largest global enterprises in the world, United Community Care Alliances Inc. has tools configured to assist neighborhoods of all sizes in devising a more efficient and proficient community. 1000+ Community Care Alliances
What if we could life coach and mentor an entire community? What if we could life coach and mentor an entire country? Virtually and in real life? What if we could mobilize neighborhoods into coordinated communities of resources and solutions?

United Community Care Alliances Inc. is launching the Community Care Alliances in over a 1000 communities across the United States. Firstly, the communities will be mobilized and assisted on-line/digitally with tools to bring health minded professionals together into community coalitions.  These coalitions are called the Healthy, Wealthy, & Wise Coalitions. Secondly, United Community Care Alliances Inc. will directly engage these communities live and in person.    

The stakeholders, people and entities that will be part of these coalitions, already exist. In most cases, the stakeholders are fragmented and do not know that other like-minded professionals and entities exist. The Community Care Alliances for every community (zip code by code, block by block) will coordinate all stakeholders into one existing alliance. Each alliance will take a “help near me” approach. 

Our research has shown that in populated zip codes, there are typically 1200 or more establishments (physical non-residential buildings). United Community Care Alliances Inc. will deploy, assist, and/or teach communities how to construct their own Community Care Alliance. United Community Care Alliances Inc. will and can facilitate the participating communities’ Community Care Alliance day to day management. The equivalent of teaching communities in a franchise approach, in essence, replicating the United Community Care Alliances Inc. business model across the United States.

1st United Community Care Alliances Inc. has already designed the framework and core components of the Community Care Alliances. The websites, software, and app are available, and updated daily. The content and curriculum have already been designed and are ready to deploy. The executive leadership and experts needed for the Care Alliance are available. Programs and products exist in limited production but are available. The relationship capital of United Community Care Alliances Inc. is strong nationally and internationally.

2nd United Community Care Alliances Inc. is eagerly pursuing strategic partners that can assist with infrastructure support, marketing, public relations, staff, subject matter experts, credibility, operations assistance, additional technology support, tactical funding, and so much more.

Virtually, nationally, and Internationally, United Community Care Alliances Inc., at this point, plans to have a full global enterprise back office infrastructure to support all the interactions between the Community Care Alliances. United Community Care Alliances Inc. data capture, analytics will generate models, and suggestions to the local Community Care Alliances to enhance all areas in the local design. Health and family first—United Community Care Alliances Inc. strategy keeps all areas of health in the center of its vision, mission, and all segments of United Community Care Alliances Inc.’s business model.  Optimistically, all areas of mental and physical health will be addressed at all time.  Addressing mental and physical health in the community are paramount to confronting issues all underserved and affluent communities face daily.  United Community Care Alliances Inc. plans to enlist every mental, medical health professional, and institutions in the country to participate in the Community Care Alliance of their local, regional, national, and international markets.

Addressing all health disparities are essential to producing and maintaining long term, viable, committed communities, while dealing with immediate short terms needs and issues of neighborhoods in those communities. United Community Care Alliances Inc. is taking a detail-oriented approach to analyzing the needs of communities based on the way residents in those communities are experiencing their quality of life. In other words, we are not trying to tell communities what to do; we are listening to what the communities say they need for their communities. The communities will be telling us how to help.

In most communities there are many moving parts (residents, businesses, faith community, finances, education structure, government involvement, etc).  Sadly, there are groups of people who think residents in challenging communities are derelict in improving their communities. Which is furthest from the truth, because most residents in these communities spend every moment trying to get out of or improve these communities. The Community Care Alliances will confront these external negative dialogs and perspectives projected on to communities as the residents having been absent in their own growth while contributing to their own demise. United Community Care Alliances Inc. will highlight the external history and influences of the past that may have neglected these communities.  On the other side, United Community Care Alliances Inc. will highlight the historic, untold, positive stories of how these same communities overcame and survived the onslaught of the lack of support.

United Community Care Alliances Inc. incisive dedication to improving communities goes beyond theory to city wide solutions, constructed to address community advancement head on.

Radius of Health—United Community Care Alliances Inc.’s deployment of the Community Care Alliances will strengthen the 3-10 miles radius of health around every person. Every person will be able to find “help near them at all time”. United Community Care Alliances Inc. will be in participants’ comfort and convenience zone of support.

There’s an app for that—United Community Care Alliances Inc.’s technology, apps, websites, programs, processes have customized design for all health stakeholders. How do you help everyone? Get everyone to help. Health disparities do not typically start inside of families and communities, they start from removal of external health resources, education, and experts in the surrounding community. Lack of access to health solutions (i.e., women services, youth services, elderly and aging services, mental health services, adequate food and nutrient options, inconvenient or non-existent medical facilities and environment grievances, just to name a few.)
  
In most cases, underserved and neglected communities suffer from redirected resources, not from its citizens being derelict in their concerns for their own health. More often than not, and so far from the truth, the assumption is underserved and neglected communities do not want any help, do not appreciate help or do not take advantage of the help offered.  It is almost impossible to solve problems from a distance. If you are not willing to sit at the table with the people you are trying to help, why would they believe you care. From experience we know, if we go to them, they will come to us.

Take an existing idea and give it a new spin or novel application; 
People do not care until they know you care! The saying absolutely holds true. Alliances and coalitions are everywhere trying to solve big problems. How do you mobilize, the mobilized? How do you organize all these alliances and coalitions into one unified force? How do you get every community to provide for itself? How do you turn on and cultivate all untapped resources, solutions, and providers within a community? There is an answer, United Community Care Alliances Inc. Community Care Alliances and Healthy, Wealthy Wise Coalitions and Champions brings everyone to the same tables.

Mobilized and coordinated communities—United Community Care Alliances Inc. has strategies to bring services within individuals and communities convenient and comfort zones. At any time, no one should be no more than a few blocks away from front line resources and relationships, hence their convenience zones. Ultimately, problems will be eliminated or dealt with directly by being part of the daily living and movements of individuals. Healthy and affluent families surround family members with 24 hours of care monitoring.  Why should we not do this for individuals in struggling communities?

Apply ideas from other fields
The Urberizing of Care—Imagine using every industry, professional field and sector of commerce, cities and communities to solve community problems together. United Community Care Alliances Inc.’s Healthy, Wealthy, & Wise Coalitions will do exactly that. Imaging every health, financial and education professional and entity contributing some asset (insights, expertise, time, information, knowledge, access, resources, relationships, funding, coaching, mentoring, guidance and leadership) to uplifting the quality of life for everyone. Yet, each person and entity will only have to give what they can contribute, hence, a coordinated community.

The identifiers (Help Near Me): Logos, shields, wrist bands, websites and apps.
Example, every member of the coalitions and alliances will be identified by having a Community Care Alliance wrist band with the area of support they are willing to provide to the Alliance. If they come from the health field, they will have health colored wrist band. In public and through marketing, the communities will know it is safe to engage someone with this band for health inquiries. A person with this band is not expected to treat or provide service, but to direct the person to local services. This process would work exactly the same for those who have financial and education experience.

For individuals’ cars, they will be able to display Community Care Alliances magnetic logo shields on their car. The shields will alert individuals in need that the person with the car can be approached for inquiry.  United Community Care Alliances Inc. will monitor all interactions through the app. United Community Care Alliances Inc. will constantly look for best practices and share results and outcomes. This will allow us to life coach and mentor every community. Every person in a community will have a minimum of three life coaches (health, wealth, personal development) if they so desire.
